While you have the freedom to make choices in my classroom, you are not always free from the consequences of those choices. While I am confident you will make the best choices in order to be successful this year, it is necessary for you to understand the consequences should you choose to violate classroom or school rules. Below is an explanation of my discipline policy. A copy of the behavior form a student will be given if s/he is in violation of policy can be found under "Classroom Documents."

- Behavior will be addressed by Mrs. Ellis

EX: “Tom, please be respectful and be quiet while Jane is speaking”

- IF behavior continues, student will be given a behavior form to fill out quietly in the hallway.

- When the form is given, student will immediately walk quietly to the hall to fill it out. Students will have no more than eight minutes to fill out the form.

- After form is filled out properly, student will return to class and give the form to Mrs. Ellis without disrupting the class further.

- If behavior continues upon return OR the form is not completed in a timely manner OR the form is not filled out properly, student will receive a referral.

- Depending on the severity of the behavior, students may be asked to complete the form immediately without previous warning.

- Behaviors deemed too severe may result in an immediate referral, call to home, and/or dismissal from class to the office.
